The format
The UK National Careers Service describes a cover letter as short, usually 3 to 5 paragraphs, and says to write a new one for every job.
- Greeting. Use the hiring manager's name if you can find it.
- Opening. The job title, where you saw it, and one line on why you want it.
- Fit. Two or three skills or results from your resume that match what the ad asks for.
- Why them. Something true you learned about the employer.
- Close. Thank them, say how to reach you, and sign off.
UK sign-off: "Yours sincerely" if you used their name, "Yours faithfully" after "Dear Sir or Madam". In the US and Canada, "Sincerely" or "Kind regards" is common.
Short example
Dear Ms Rivera,
I am applying for the Customer Service Advisor role listed on your careers page. I have spent 3 years answering phone and email questions at Example Telecom, where I handled billing queries and logged each case in Zendesk.
Your ad asks for calm phone manner and accurate notes. In my current role I take about 40 calls a day and my team lead reviews my case notes weekly.
Thank you for considering my application. I can be reached at 555-0100.
Sincerely,
Sam Example

Do I need a cover letter if it is optional?
It can help explain fit, a career change or a gap. If you send one, write it for that job.
Who do I address it to?
The hiring manager by name if you can find it. If not, "Dear Hiring Manager" (US, Canada) or "Dear Sir or Madam" (UK) works.
